Many parents first hear the word "adenoids" when their child's snoring becomes impossible to ignore. Adenoids are a small pad of tissue at the back of the nose, above the roof of the mouth. How enlarged adenoids are diagnosed

The diagnosis is based on symptoms and examination. In older children, a short nasal endoscopy with a thin camera gives a direct view. In younger children, an X-ray of the side of the neck (lateral X-ray of the nasopharynx) shows the size of the adenoids. A hearing test and tympanometry are done if ear problems are suspected.

Treatment without surgery

Mild cases are first treated with saline drops and steroid nasal sprays for six to eight weeks. Treating allergy, if present, also helps. Many children improve, and adenoids naturally shrink by around 7–8 years of age.

When is adenoidectomy advised?

  • Persistent nasal blockage and mouth breathing despite medicines
  • Snoring with signs of sleep apnea
  • Repeated ear infections or long-standing glue ear
  • Repeated sinus infections in children

Adenoidectomy is often combined with tonsillectomy when the tonsils are also large, or with grommet insertion when there is fluid behind the eardrum.

The procedure

Adenoidectomy is done through the mouth under general anaesthesia, with no outside cuts. It usually takes about 20–30 minutes. Most children go home the same day. Endoscopic techniques allow the adenoid tissue to be seen clearly and removed precisely.

Recovery

  • Mild throat pain and a blocked nose for a few days are normal.
  • Some children have bad breath or a nasal voice for a week or two.
  • Soft food and plenty of fluids help.
  • Most children return to school within about a week.
  • Any bleeding from the nose or mouth should be reported immediately.

Parents often notice quieter sleep, less mouth breathing and better appetite within a few weeks.

Studies have not shown a meaningful effect on immunity. Other immune tissues in the body continue to protect the child.
